WHO QUALIFIES FOR A MEDICAL CANNABIS CARD?

Seasonal or permanent residents of Florida (18 years or older) with an eligible medical condition such as anxiety, insomnia, PTSD, Crohn's, alcohol/opioid use disorder, pain, cancer, HIV or epilepsy for example.

If you are coming in for a NEW certification or seeing Dr. Shaw for the FIRST time, you MUST BE SEEN IN PERSON. All other visits are able to be done remotely.

Founded in 2021, CGS Physician Services, LLC began with a vision of being a catalyst for positive change in the world of cannabis.
