I went to this place to get information on how to get my card. I walked in and there was no one at counter. But there was 2 lady's standing there. I looked around and pretty soon one of the lady's turn to me and said u need something. I asked her if she had a form that I needed for my doctor to sign. Her response was, your doctor can print it off the site. That's what I have to do. She was so rude I just turned and told her I could get it elsewhere. And I walked out with her mumbling under her breath. Will never go back there again.
