For Dr. Jay that # is: ten digits 43015 followed by 01404 (unable to enter normally in this review due to phone number conflict) 3) After you submit to state, drop me an email, saying something like "certification requested" 4) I will then contact you to video-chat, discuss medical issues, and arrange on-line payment (my charge $100) 5) After medical confirmation you will receive an email, which you can use at the provisioning center until your card arrives in the mail. 6) In conclusion, without leaving your place of social isolation, for cost of $140+product cost. You can "see doctor" and get card online, and have product delivered to your door, usually in the same day.
