All you need to provide from your doctor is a referral. These people ask for your medical records as well. You may think "I've come this far and I really need my medicine." No. This is your private information to be shared only between you and your physician. No one has a right to this but you. Not only do they just ask but I got a call from a half confused, half entertained assistant to my doctor asking why the heck this clinic was asking for my medical records. My doctor wondered who'd have the audacity to request them and asked her assistant to call me. She didn't ask if she could send my records but she did ask if she could shred the request. I gave it. Good people, do not ever allow anyone access to his information! What if your referring doctor is a psychiatrist? Part of CCC's request was "physician's notes." What happens to your precious data when they go out of business? They're not the government held to high standards of privacy.
Grungy scuzzy looking place. Never "saw" a Dr.
Worst place for keeping an appointment time. Worse place to try to access or park.
So many other options.
These guys need to revamp & do things more professionally.

I have dealt with this place & it's important for people to know that there are better places out there to get your medical marijuana license. The Dr they have refuses to listen to patients & he refuses to follow the law. The Dr at this clinic is not at all interested in helping people & he restricts patients use of the medication & refuses to prescribe if you don't agree to use the LP that this Dr chooses for you. After 2 visits & trying to give them a chance I went back to my old Dr in Toronto who treats patients properly. Also I was told by a reliable source that they are under investigation by the college for how they treat their patients so I recommend staying away.
