Please email a copy of your medical records, which you can obtain from your own family doctor if you don’t already have them, or a letter from your doctor stating your medical condition and what treatments you have tried. Alternatively, we have a physician referral form under the Pre-Qualify tab, optional forms. Additionally, our doctors also accept a copy of your patient profile from your local pharmacy showing all your medications. We can also accept a note from a chiropractor or a naturopathic doctor. Please email a copy of your photo ID’s, either a copy of your driving license, Health Card or passport. Scanned and emailed photos work better than faxed.
